Engaging with Service Users and Carers
All learning disabilities service users now complete personalised Health Action Planning Packs (HAPPs) with the help of carers, relatives and health care professionals.
Each easy-read pack contains essential information about the service user, including allergies and immediate risks, so everyone involved in their care can ensure their needs are met. The pack includes details of medication, healthy eating advice and a hospital assessment in case they are admitted to hospital as an emergency.
Service users, carers, support staff and professionals are all represented on our Health Action Group. The group looks at health inequalities and health-related issues and feeds back to the Learning Disability Partnership Board with recommendations for action.
