Intensive Health Outreach Team
Our Intensive Health Outreach Team provides assessment and treatment for adults with profound and multiple learning disabilities and other vulnerable adults with associated health problems.
The service is predominantly community based with the option of a two bedded admission facility when required.
Interventions are implemented in the most appropriate environment to meet the clients needs.
The service supports the most vulnerable people with a learning disability. This includes people with profound and multiple disabilities and older people with learning disabilities whose health needs require a level and intensity of specialist intervention that is not currently provided by a Community Learning Disability Team (CLDT).
