MM2G - Memory Assessment Service
Our Memory Assessment Service offers an assessment, dementia diagnosis and treatment service for people living in Gloucestershire aged 18 years of age and over.
How to access the Memory Assessment Service
Most people are referred to our Memory Assessment Service by their GP. You can also refer yourself directly to the Service by calling us on 0800 694 8800. We would advise that you seek help from your GP in the first instance to rule out other possible causes for memory loss.
If you feel unable to talk to your GP about your own or a family member's memory problems, contact us directly. We will provide advice on next steps which may include arranging an appointment with the memory assessment team.
MM2G - Community Dementia Nursing Service
Community Dementia Nurses provide support to GP practices in the diagnosis, management and treatment of dementia. They are also responsible for monitoring dementia medications. The service also provides short term support to people with dementia and their carers to address issues and immediate care needs arising from their diagnosis.

Community Dementia Nurses work closely with the Dementia Adviser Service that is delivered through the Alzheimer’s Society. Dementia Advisers help anyone diagnosed with dementia and their families to get the information they need about the illness and where to find the most appropriate services in their local area.

MM2G – Information and Education Service
Information and Education Service can offer information and advice to people worried about memory, people with dementia and carers of people with dementia. The service also provides a countywide programme of information sessions to support people with dementia and carers to understand their diagnosis and live as well as possible with dementia.
