Mental Health Nurse Liaison Service
The Mental Health Nurse Liaison Team provides educational and clinical support in the care and treatment of patients with dementia and related conditions. The patients will have been admitted to Cheltenham General Hospital, Gloucestershire Royal Hospital and the Community Hospitals across the county.
Care Home Support Team
This is a multi-agency partnership to provide support for care homes in Gloucestershire. These teams include mental health nurses and other professionals whose role is to support improvements in residential and nursing home care. The role also includes improving the network of care and involvement of people who live in care homes.
Intermediate Care (Intensive Support at home)
Community Mental Health Nurses (and support workers / Rehab Assistants) work as part of a social care team to prevent admission and facilitate hospital discharge for people with dementia.
