Services for Working Age Adults

 Who uses our services?

The service primarily provides treatment and care for service users between the ages of 18-65 and whose circumstances dictate the support of mental health professionals. 2gether offers a comprehensive range of services including Community Rehabilitation and Recovery Services, Crisis Home Treatment, Early Intervention in Psychosis and In-Patient Services among others.


How can we help?

Mental ill health effects 1 in 4 of the general population in England. It can be a very frightening experience where you can feel isolated from family and friends, feel you have nothing left to offer those around you and often people do not know who to go to for help.

The services work with users and carers to develop individualised care plans to help the person, with support from others, to work towards bringing back stability to their lives, improving a sense of self worth and where necessary maintaining or improving independence.

Services available address a wide spectrum of need beginning with mild anxiety and depression to more complex and enduring mental illness.

Our Team

There are many different people you may see in working age adult services, these may include:

    • Consultant Psychiatrists, Senior Registrars and other medical staff
    • Mental Health Nurses and Support Workers
    • Allied Health Professional which include, Psychologists, Occupational Therapists, Physio and Sports Therapists and Arts Therapists.
    • Social Care Specialists

All of the people are there to support you, your family and carers and provide the services that you or the person you care for require.

How do people access our service?

People generally access our services via their family doctor usually referred to as their GP however for some services such as our ‘Lets Talk’ service it is possible to self refer. The links on the right tell you more about referral for each of our services.

Assessments

Most People referred to our service have a standard core assessment looking at all your needs. A general assessment can take between 1 and 2 hours and covers such things as your current situation, problems, health difficulties, social circumstances and strengths. Further assessments are normally conducted to look at specific problem areas in a more detailed way.

We also offer carers assessments to understand these needs and make recommendations for support. If you feel you need one please do not hesitate to ask your mental health practitioner.

What happens during your treatment?

After assessment, the person coordinating your care normally agrees the care plan with you. The plan will identify what treatment, help and support you will be having, for how long and with whom. There will be regular opportunities for the plan to be reviewed. The care plan will normally involve contact with one or more professionals for a period of time.
