Learning Disabilities Team
The Herefordshire Adult Community Learning Disability Team (CLDT) works with those who have a learning disability and a health need. The team can assist with both physical and mental health.
The team can help people living with a learning disability to make informed choices about their health and wellbeing, and to live as safely and independently as possible and have a healthy lifestyle.
The team comprises of learning disability nurses, an occupational therapist, a physiotherapist and physiotherapy assistant, speech and language therapists, psychologists and a consultant psychiatrist.
The team also works with family members of those with a learning disability, GPs, hospitals, social workers and colleges, as well as other people who may need assistance to support someone with a learning disability.
The service has an open referral system and therefore will accept referrals from anyone including self-referrals, family members, carers and provider services. Download the referral form here.
